There should be a contact number on your insurance card or with the policy. Generally you can call them and get approval in advance for surgery. If you visit your doctor their office should be able to call for you. If you are trying to find out before visiting the doctor then call yourself or have your mom call for you. They will have a department that handles approval. Also, consider if there are other things you might do for your back. Physical therapy and certain exercises. I'm sure you must be really uncomfortable but these surgeries aren't without risks and while you think it's awful now, you might want to feed your babies in the future and be unable to because of surgery. fed babies are healthier and smarter. Mother's that feed are also more intelligent. (I know if you are larger than average and uncomfortable this isn't what you want to hear...but you need to explore all options.) -all the best.
